The role of autoimmunity and the loss of immune tolerance to own proteins have a significant place in the current ideas about the aetiology and pathogenesis of systemic lupus erythematosus (SLE). The disease is characterized by broad-spectrum organ manifestations, multiple autoantibodies and multifactorial immunological pathogenesis. Lupus nephropathy is one of the most severe, significant and common manifestations of SLE. Patients with lupus nephritis (LN) have higher morbidity and mortality than SLE patients without renal involvement. LN affects the quality of life of patients and the prognosis of SLE. Treatment is almost always long-term, relatively often associated with many side effects for the patient and his relatives and has significant financial dimensions. Given the severity of LN, which often leads to developing renal failure, and determines the long-term prognosis in patients, there has been an intense search for biomarkers for early diagnosis, more accurate assessment of disease activity, severity and response to therapy. The complement system, part of the innate immune defence, plays a significant role in the immunopathogenesis of SLE and LN. This role is seen as twofold: on the one hand, activation of the complement cascade involving the mechanisms of innate and acquired immunity leads to the development of inflammation, the immune response to autoantigens and ultimately persistent tissue damage, and on the other - insufficient activation of the complement cascade leads to impaired clearance of immune complexes and apoptotic cells with their accumulation and development of autoimmunity. Autoantibodies to complement components and regulators play a significant role in the acquired functional deficits of the complement cascade in SLE and LN. The most important anti-complement autoantibodies that have a pathogenetic role in developing SLE and mainly LN are anti-C1q antibodies. Other autoantibodies against components of the complement system described in SLE and LN have not been sufficiently studied, and their pathogenetic and clinical significance at this stage is not fully understood.
