Obesity is a chronic, multifactorial disease in which the accumulation of body fat leads to adipose tissue dysfunction and the development of dyslipidemia, type 2 diabetes, atherosclerosis, and arterial hypertension—key components of the metabolic syndrome. Endothelial dysfunction, insulin resistance, chronic inflammation, and oxidative stress play crucial roles in the pathogenesis of the associated cardiovascular complications. Based on this, we formulated the hypothesis that in obesity, elevated inflammatory and oxidative markers lead to more pronounced endothelial and cardiomyocyte dysfunction, thereby promoting the progression of cardiovascular diseases. Research in this field is challenging due to the complexity of the mechanisms involved in endothelial and myocardial injury. The analysis of endothelial-protective and antioxidant agents may enhance our understanding of the processes related to the protection and partial restoration of impaired function. Despite existing data, the literature remains limited regarding the reversible manifestations of endothelial dysfunction. A promising approach is the use of experimental animal models to reproduce these alterations. The present study aims to elucidate the molecular mechanisms underlying endothelial and cardiomyocyte damage and the potential antioxidant protection, as well as to outline opportunities for future therapeutic interventions aimed at improving vascular and cardiac function in obesity.
