Allogeneic haematopoietic stem cell transplantation (allo-HSCT) is an established therapeutic approach for a range of malignant and benign haematological disorders. Despite significant advances in the field of transplantation medicine, this method remains associated with a number of complications – graft-versus-host disease (GVHD), infectious complications (viral, bacterial, fungal), non-infectious complications (dysfunction of various organs and systems), and delayed or incomplete recovery of the immune system. Following allo-HSCT, immune system reconstitution is a complex and prolonged process, which has critical importance for patient prognosis. Lymphocyte subpopulations play a key role in the immune regulation of recipients, and an imbalance in their number and function can lead to severe consequences – GVHD, relapse of the underlying disease, infectious and non-infectious complications. Understanding the dynamics of these subpopulations in the post-transplant period is essential for improving therapeutic strategies and patient outcomes.
