Въпреки значителния напредък в областта на трансплантационната медицина, този метод е свързан с редица усложнения – болест на присадката срещу приемателя (graft versus host disease; GVHD), инфекциозни усложнения (вирусни, бактериални, микотични), неинфекциозни усложнения (дисфунцкия на различни органи и системи) и забавено и непълно възстановяване на имунната система. След ало-СКТ, възстановяването на имунната система е сложен и продължителен процес, който има критично значение за прогнозата на пациентите. Лимфоцитните субпопулации играят ключова роля в имунорегулацията на реципиентите, като дисбалансът в тяхната численост и функция води до тежки последствия – GVHD, рецидив на основното заболяване, инфекциозни и неинфекциозни усложнения. Разбирането на динамиката на тези субпопулации в посттрансплантационния период е от съществено значение за подобряване на терапевтичните стратегии и прогнозата на пациентите. Проучванията върху кинетиката на имунното възстановяване след ало – СКТ разкриха ключови аспекти в основните принципи, на които лежи този терапевтичен метод. През последните години изследванията в тази област се развиха от чисто описателни проучвания до високо динамична и иновативна сфера, която оформя бъдещето на ало – СКТ. Бъдещите проучвания ще бъдат насочени към разработване на по – прецизни модели за прогнозиране на посттрансплантационните усложнения, отчитайки сложните клетъчни взаимодействия и включвайки допълнителни фактори като тип присадка, вид донор, манипулация на присадката, степен на съвместимост между реципиент и донор.
