ΕΠΙΘΗΛΑΚΗ ΕΚΦΡΑΣΗ ΑΝΤΙΓΟΝΩΝ HLA-DR ΚΑΙ ΛΕΜΦΟΚΥΤΤΑΡΙΚΟΙ ΥΠΟΠΛΥΘΥΣΜΟΙ ΤΟΥ ΓΑΣΤΡΙΚΟΥ ΒΛΕΝΟΓΟΥ ΣΕ ΧΡΟΝΙΑ ΓΑΣΤΡΙΤΙΔΑ ΤΥΠΟΥ - Β: παθογενετική συσχέτιση
Abstract
dc:descriptionLymphocyte subpopulations (Β,ΤψΤ8) monocytes/macrophages (LeuMB.KiMS) and HLA-DR antigens were studied immunohistochemically in frozen sections from 32 antral and 37 fundal biopsies of type-B chronic gastritis. In 1/4 of the cases, areas of intestinal metaplasia were found in paraffin and/or frozen sections. The topographical distribution of nononuclear cell infiltrates was closely related to the histological subtypes of the gastritis. In all cases the monuclears were densely infiltrating the lanina propria around the crypts. B-lymhocytes were less numerous than the other nononuclear cells and represented in most cases less than 50 % of the total T-cells. Helper/inducer T-lymphocytes were the more frequent cell type in extraepithelial areas of lamina propria. In the majority of the cases examined their relative numbers compared to suppressor/cylotoxic T-lymphocytes ranged from 1.2:1 to 2:1. Helper/inducer T-lymphocytes were infrequently seen within epithelial cells. The majority of IEL cells expressed the suppressor/cytotoxic phenotype. The distribution and number of nonocytes/histiocytes(Leuf*15, and KiN5 positive) paralled those of T-lymphocytes and especially of the helper T-lymphocytes. They were not found intraepithelial!y. In areas of intestinal metaplasia B-lyemphocytes T-lyemphocytes and monocytes/histiocytes were very rare. HLA-DR positive epithelial cells were found in all cases of chronic gastritis. The percentages of HLA-DR positive epithelia cells depended on the severity of gastric inflamation and there was a relationship with the number of T-cells especially of the Τ 4 cell phenotype. In the foci of intestinal metaplasia the epithelial cells were practically 123 negative for HLA-DR antigens. Our findings support the notion that the etiological factors either directly evoke an abberrant epithelial HLA-DR expression or they cause a local immune inflammation and the production of interferon -x results in aberrant epithelial HLA-DR expression. In both cases an innunopathological mechanism is elicited. Recently a bacterial named Campylobacter pyloridis has been found in 90 % of type -B chronic gastritis cases and has been proposed as the aetiological factor. The distribution of epithelial HLA-DR expression in our material paralles the distribution of C pyloridis reported by others. These bacteria were absent in the intestinal metaplastic areas where lymphocytic infiltrates and aberrant HLA-DR expression was minimal. It can be speculated that eithet C-pyloridis bacterial products directly induce, as lectins do, epithelial HLA-DR expression or C-pyloridis absorbed by the specific antibacterial antibodies evoke a local immunological reaction, resulting in x-interferon production wich subsequently stimulate the epithelial cells to express HLA-DR antigen and thus an autiommune process begins.
