Cytokines and Health Related Quality of Life: A longitudinal study in breast cancer survivors

Objective: Cytokine-driven inflammation is known to drive many conditions which impair a person's Health Related Quality of Life (HRQoL). We aimed to understand if self-reported HRQoL among breast cancer survivors is associated with circulating cytokines. We examined the cross-sectional associations between cytokines and HRQoL at the time of diagnosis before treatment and one year later post-treatment. We examined if cytokine levels at the time of diagnosis were associated with HRQoL at one year, and also if change in cytokines from at diagnosis to one year post-diagnosis were associated with the HRQoL at one year. Methods: Women in the After Breast Cancer study completed an epidemiological questionnaire which included a HRQoL assessment (Short Form-36) at breast cancer diagnosis before treatment (n=434) and after one year (n=326). Blood samples collected at diagnosis and at one year post- diagnosis were analyzed for twenty five circulating cytokines using Luminex bead-based assay. Odds ratios (ORs) and 95% confidence intervals (CIs) for poor HRQoL (≤median) among tertiles of cytokine concentrations were estimated with multivariable logistic regression. Least Square means and 95% confidence intervals (CIs) for continuous measures of HRQoL were also estimated with linear regression by tertiles of cytokines. Trend analysis was performed to check if there is a positive trend of the HRQoL scores from the lowest tertile to the highest in both the logistic and linear regression models. All analyses were adjusted for age, menopausal status, race, BMI, smoking status, education, breast cancer stage, and estrogen receptor status.
