Comparison study of Taurolidin, Talc, Tetracycline and Bleomycin Pleurodesis in the Rabbit Model : Analysis of the Taurolidin's effectiveness

dc:descriptionBackground and PURPOSE: Management of recurrent spontaneous pneumothorax or symptomatic pleural effusion often uses chemical pleurodesis. Numerous sclerosing agents such as Talc, Tetracycline, Doxycycline and Bleomycin have been used to induce pleurodesis. These agents have some limits of these use. Therefore some have used Taurolindin (Taurolineⓡ) as chemical irritant which is inexpensive, nontoxic and easy to deal. However it has not been investigated the effectiveness of sclerosing agent. We classified the effects of each chemical irritant and inspect the effectiveness of Taurolidin in pleurodesis. MATERIAL AND METHODS: Administrated Agents including Talc, Tetracycline, Taurolidin and Bleomycin were given thorough a 10 Fr. silastic trocar catheter to 40 Rabbits. 2 rabbits as control had been only inserted using a 10 Fr. silastic trocar catheter at 30 days after pleurodesis pleural surfaces were graded by gross observation and histologic examination. These scores have been compared. RESULTS: Overall gross necropsy score was Talc 2.64 ± 0.51, Tetracycline 1.86 ± 0.75, Taurolidin 1.78 ± 0.56 and Bleomycin 1.21 ± 023. Overall histologic score of visceral pleura was Talc 2.05 ± 0.97, Tetracycline 1.85 ± 0.89, Taurolidin 1.80 ± 0.63 and Bleomycin 1.07 ± 0.36. Overall histologic score of parietal pleura was Talc 1.98 ± 0.91, Taurolidin 1.67 ± 0.60, Tetracycline 1.50 ± 1.04 and Bleomycin 1.26 ± 0.46. Effect of Taurolidin pleurodesis was no different to other sclerosing agents. Only Bleomycin was less effect compared to Talc in gross score and histologic score of the parietal pleura (p < 0.05). CONCLUSION: In our study, effective sclerosing agents were Talc, Tetracycline, Taurolidin and Bleomycin in order. Taurolidin seems effective sclerosing agent because of no statistic difference to other effective agents. Talc was the most effective agent but had no statistic significance. Taurolidin should be more investigated for its effects compared with other agents in clinical trials.
