Increased BCA-1 Expression and Attenuated Apoptosis in Epithelial Cells in Helicobacter pylori-associated Low Grade Gastric MALT Lymphogenesis

BACKGROUND: Helicobacter pylori(H. pylori) infection is one of the major causes of the pathogenesis of low grade gastric MALT lymphoma(MALToma), but the reason why only a few patients with H. pylori infection developed MALToma was not clearly explained yet. Here, we provide novel finding that up-regulated expressions of B cell attracting chemokine-1(BCA-1) and deranged apoptosis in gastric epithelial cells are causally involved in the transformation of H. pylori-associated gastritis to MALT lymphoma. MATERIALS AND METHOD: Using tissue blocks from 20 patients with H. pylori-positive gastritis, 39 patients with H. pylori-positive low grade MALT lymphoma, and 18 patients with H. pylori-negative gastritis, in situ hybridization for BCA-1 and its receptor, CXCR5, and TUNEL staining for apoptosis were done, respectively. We tested a change of BCA-1 and CXCR5 using in situ hybidization after H. pylori eradication in 28 patients, after which the complete remissions of MALToma were achieved. Also, we evaluated the direct influence of H. pylori infection on expression of BCA-1 and CXCR5 mRNA in gastric epithelium using RT-PCR. RESULTS: The expression of BCA-1 and CXCR5 was significantly higher in patients with H. pylori-positive MALToma than H. pylori-positive gastritis or H. pylori-negative gastritis, of which expressions were significantly reduced in the remission of MALToma after the successful eradication of H. pylori. Significant attenuations of epithelial apoptosis were observed in H. pylori-associated MALToma compared to abundant apoptotic activities after H. pylori infection. H. pylori infection directly induced the expression of BCA-1 in the cultured gastric epithelial cells. CONCLUSION: The increased BCA-1 expression and dysregulated apoptosis in H. pylori-infected gastric epithelium might contribute to development of MALT lymphoma.
