Die Expression der Matrix-Metalloproteinasen MMP-2 und MMP-9 beim Kolon- und Rektumkarzinom

Introduction: Colorectal carcinoma is one of the most common tumors of the human being. Prognosis depends very much on the tumor state, invasion and metastasis. The investigation of invasion and metastasis are therefore of great relevance. Metastasation includes several important steps. One of these steps is the degradation of the extracellular matrix (ECM), which surrounds the tumor. Among other things, proteinases (for example matrix- metalloproteinases) are involved in this process. In tumor progression of the colorectal carcinoma, the matrix-metalloproteinases MMP-2 and MMP-9 play a special role. In this study we investigated the expression of MMP-2 and MMP-9 and their importance for therapy and prognosis of colon- and rectal carcinoma.Methods: Biopsy samples of tumor and distant normal mucosa from 20 patients with colon- and from 9 with rectum carcinoma were obtained intraoperative. They were shock-frozen and after that analyzed by gelatin zymography and western blot. Gelatin zymography allows to detect active enzyme in a qualitative and semi-quantitative way. After a densitometric analysis of the expression of MMP-2 and MMP-9 in all tissues, MMP activity of the carcinomatous tissue was compared to the activity of the healthy mucosal sample of the same patient. By using western blot analysis, we could perform a specific identification of the metalloproteinases.Results: We found a significant increase of MMP-2 and MMP-9 in cancerous tissue in all cases (P<0,05) compared with the corresponding healthy tissue. In addition, comparing rectal and colon carcinomas, we observed differences in MMP activity. MMP-9 expression was much more striking in colon carcinomas than in rectal carcinomas, whereas in rectal carcinomas there was mainly an increase of active MMP-2.Conclusion: Matrix-metalloproteinases MMP-2 and MMP-9 are increased in colorectal carcinomas. On one hand they seem to be suitable sensitive prognostic factors. On the other hand gelatinase expression is not very specific. But Inhibition of MMP-2 and MMP-9 could play an important role in therapeutic intervention of colorectal carcinoma. Taking into account earlier studies, this is the first report showing separate asservation, evaluation and comparison of colon and rectal carcinomas. The Expression of MMP-2 and MMP-9 in colon and rectal carcinomas seems to be different. This difference should be further investigated in studies including bigger amounts of patients. A differential expression of matrix-metalloproteinases could result in a different therapeutic procedure of colon and rectal carcinomas.
