Auswirkungen von Propofol und Fentanyl auf die gastroduodenale Motilität bei Schweinen gemessen mit der multiplen intraluminalen Impedanzometrie

dc:descriptionDysfunctions of gastrointestinal motility are still a major expense in intensive care. This study was performed in order to determine the effects of Propofol and Fentanyl on postprandial gastroduodenal motility of awake pigs using the intraluminal impedance technique. After conditioning the pigs for nine days, they were instrumented with a central venous catheter, a percutaneous enterogastrostomy (PEG) and an impedance catheter, which was introduced via the PEG into the duodenum through endoscopy. Over the following 3 d, duodenal motility was measured for 8-hour periods. Measurements were taken on each subject under 3 different sets of conditions: in the conscious unrestrained pig, during Propofol sedation, and during sedation with propofol-fentanyl. The following parameters could be detected by impedancometry; duration of MMC-cycle, duration of the Phase I-III, frequency of the Bolus-Transport-incidences (BTE), duration of the feeding patterns and contraction frequency of the stomach. Thereby, a reduction of the feeding pattern and Phase II was detected when sedating with Propofol as well as sedating with Propofol and Fentanyl. In addition, a significant prolongation of Phase I arose while concurrent reduction of the contraction frequency of the stomach, not only with the combination of Propofol and Fentanyl, but under administration of Propofol exclusively as well. Altogether, the results of this study indicate an impact on the gastroduodenal motility phases of pigs under sole administration of Propofol, as well as combining Propofol and Fentanyl. The effect on the motility phases could give reasons for a clinically delayed gastric emptying. By means of evaluation studies with human beings, future starting points for intensive care monitoring could yield that enable precocious detection of motility dysfunctions thus enabling the prevention and treatment respectively of accordant complications. By analyzing the effect of particular active ingredients on the duration of the separate phases, it would become apparent whether new starting points for a medicamentous impact on dysfunctions of gastric emptying are caused by the active ingredients.
