Veränderung der gesundheitsbezogenen Lebensqualität vor und nach interventioneller radiologischer Therapie bei Patienten mit angeborenen Gefäßmalformationen
Abstract
dc:descriptionWith the "Medical Outcome Study Short form 36 Health status Questionaire” briefly SF-36, the health-referred quality of life in patients with vascular malformations before and after a interventional radiological therapy was examined. It had to be clarified, how the health-referred quality of life and/or which dimensions of the quality of life changes after a interventional radiological therapy in patients with venous and arteriovenous malformations. 46 patients (31 f, 15 m, average age 32,2±8,7years) were accepted to the study, 41 patients were accepted to the therapy arm of the study and were treated with an interventional radiological therapy. 12 patients had an arteriovenous malformation wich, in all cases, was treated with a transluminal embolisation. Two Patients additionally had an LITT. 29 patients had a venous or lymphatic malformation, these were in 27 cases treated with an LITT, in 2 cases with sclerotherapy. 7 patients had an embolisation to take place to close small arterial Feeder before a LITT. The SF-36 values were seized before and after the therapy by questionnaires. The follow up averaged 1,9±1,5 years. At the beginning of study all the SF-36 values of all patients deviates in the scales physical operability (45,11), physical role function (46,56), general health perception (47,39) and pain (40,91) of the population of standard (50,00), i.e. patients with vessel malformations have a worse health-referred quality of life than the population of standard. Particularly patients with venous malformations show a significantly worse value before a therapy in the pain scale (36,94) opposite patients with arteriovenous malformation (46.94). Exactly these patients with venous malformatios have a significantly improved value in the pain scale (48.08, p=2,69x10-6) after therapy, i.e. fewer pain and thus a clear improvement of the health-referred quality of life. Likewise is a significant improvement the physical operability (from 46.04 to 49,30, p=0,039) and the general health perception with patients with venous malformations (from 48,38 to 51.44. p=0,033). In patients with arteriovenous malformation a small change of the individual scales can be attained, these was however not statistically significant by a therapy. Patients with venous malformation it came to a statistically significant (p=0,011), proportional volume reduction of the malformations of 57,5±27,6%.
