In recent years, there has been a lot of focus on mild stroke research. Mild stroke is usually excluded from the IVT treatment protocol due to the potential benign prognosis and the potential risk of bleeding, although some of these patients may have poor outcomes. There are also 'invisible' stroke symptoms after stroke - those that are not detected by the NIHSS. This review provides an overview of the NIHSS and mRS scales, their relevance in clinical practice, and the invisible symptoms that are missed by the NIHSS. Aim of the study: to review and evaluate the invisible/understudied stroke symptoms and their practical relevance in stroke clinical practice. Objectives. To review the NIHSS and mRS scales in the literature. 2. To review the literature for studies that investigate stroke symptoms that are not evaluated/understudied in the NIHSS and mRS scales. 3. To assess the practical relevance of unseen/understudied stroke symptoms in clinical practice and make suggestions. Method ⎯ Analysis of the scientific literature. Databases used ⎯ PubMed, Google scholar. Inclusion criteria: open access publication of scientific literature; articles from 2014-2024; articles in English. Exclusion criteria: incomplete text of the publication; paid publication. Conclusion: The review of the literature found that invisible stroke symptoms, although most often present in minor stroke, cause long-term changes in the patient's quality of life and outcome. Particular attention should be paid to neuropsychiatric disorders, as they correlate strongly with mortality. There is a lack of data and studies on the use of thrombolysis in patients with symptoms other than NIHSS. More targeted research on this topic is needed before detailed recommendations can be made.
