Abstract
dc:descriptionAcute drug intoxication is a major problem worldwide, with antidepressants, neuroleptics, and cardiovascular drugs being the most commonly overdosed drugs. Most of these do not have a specific antidote and cause serious and sometimes fatal cardiovascular and neurological complications, necessitating the introduction into the routine practice of more effective treatments such as intravenous lipid emulsions (LE). Initially, LEs were developed to meet the necessity for essential fatty acids in patients receiving parenteral nutrition. In the last 15 years, a new effect of LE has been discovered, initially for the treatment of local anesthetic systemic toxicity (LAST), and subsequently, their ability to capture lipophilic drugs and to retrieve them from target organs such as heart and brain and thus reduce their toxicity, has been demonstrated. (Weinberg, 1998). The questions that remain unresolved are whether LEs have cardioprotective effect when administered alone, what is the dosage of acute exogenous intoxications, different from LAST and ambiguity regarding the complications of high- dose and high- speed LE administration. The aim of the study is to find complete information on LE, composition, mechanism of action, dosage, infusion rate, side effects, related to rapid administration of LE, to analyse the effects of incident clinical administration of LE in acute Verapamil poisoning in humans and to determine whether LEs have a cardioprotective effect in an experiment with rats. Our study would have significant scientific and applied value for both clinical practice and experimental medicine. This knowledge will contribute to a better understanding of the mechanisms of action of LE, the risks, associated with its use, and also improve the quality and effectiveness of treatment in patients with acute intoxications with Verapamil and other lipophilic drugs. We aim to investigate the cardioprotective effect of LE as an innovative method for the treatment of acute exogenous lipophilic drug intoxications in induced heart damage, dose determination in shock or hemodynamic instability, monitoring of side effects associated with rapid administration and possible overdose.
