This thesis presents an in-depth retrospective analysis of the microbiological profile of purulent-septic diseases in the maxillofacial region (MFR), treated at St. Marina University Hospital – Varna city, Bulgaria, over a ten-year period (2015–2024). The aim is to create an up-to-date "microbiological map" for the region, supporting the optimisation of local therapeutic protocols and the fight against antimicrobial resistance. The study employs a retrospective, observational, and descriptive design, based on the analysis of 1735 microbiological results from samples (pus, aspirates, wound secretions) obtained from patients with a clinical diagnosis of MFR abscess or phlegmon. Each isolated pathogenic microorganism was recorded as a separate isolate. Samples were divided into odontogenic (n=1033) and non-odontogenic (n=702) infections, and temporal analysis was conducted across two 5-year periods (2015-2019 and 2020-2024). Microbiological methods include standard inoculation onto culture media (blood agar, chocolate agar) and incubation under aerobic, microaerophilic, and anaerobic conditions, followed by identification through macro- and micromorphological characteristics, Gram staining, and biochemical tests. Statistical analysis utilised descriptive statistics (absolute frequencies and relative proportions). The goals include: determining the overall etiological structure of infections; conducting a comparative analysis of the microbiological profile in odontogenic and non-odontogenic origins; tracking the dynamics of the main causative agents over time (before and after the Covid-19 pandemic); and analysing the frequency of polymicrobial infections and samples without a proven causative agent, discussing the possible reasons. The expected results will provide valuable scientific information on dominant pathogens, trends in their distribution, etiological differences, and diagnostic challenges, contributing to the improvement of local therapeutic practice and the fight against antimicrobial resistance.
