Biomarkers for Personalised Treatment Approach in Patients with Multiple Myeloma // Биомаркери за персонализиран подход при подбора на лечение при пациенти с миеломна болест
Abstract
dc:descriptionThe thesis aims to evaluate the role of circulating microRNAs (miRs) as diagnostic and prognostic biomarkers in multiple myeloma (MM). The cohort includes 56 untreated MM patients and 12 healthy controls, with a focus on the quantitative determination of expression levels of five selected miRs: miR-199a-5p, miR-126-5p, miR-497-5p, miR-214-3p, and miR-373-3p. The results show a significant decrease in miR-126-5p and miR-199a-5p in newly diagnosed MM patients compared to healthy controls, suggesting their potential utility as early diagnostic biomarkers. Longitudinal analysis demonstrates a significant increase in miR-126-5p levels after therapy, correlating with treatment response. In contrast, elevated expression levels of miR-214-3p and miR-497-5p are associated with unfavourable clinical outcomes, indicating their potential as prognostic indicators. Receiver operating characteristic (ROC) curve analyses reveal that miR-199a-5p has high sensitivity (87.04%) for distinguishing MM patients from controls, whereas miR-126-5p exhibits balanced diagnostic accuracy. These findings support integrating miR profiling into clinical practice to improve risk assessment and facilitate personalised therapeutic approaches in MM, ultimately aiming to enhance patient survival and quality of life.
