The thesis aims to confirm that during treatment of edentulous lower jaw with fixed implant-retained rigid prostheses, the mandibular flexure during function has a negative influence of the peri-implant bone, and along with that to create an algorithm for the quantity and positioning of the implants and the type of the prostheses in order to prevent the consequences of the manbidular deformation. The aim can be achieved by resolving the following tasks: 1. A study on the mandibular flexure during function by creating a 3D computer finite element analysis (FEA). 2. A study on the stress distribution in the peri-implant bone around the most distal implants using SolidWorks design and 3D finite element analysis. 3. An in vivo study on the mandibular flexure by analysis of the distances between #37 and #47 in a state of relaxed and contracted masticatory muscles. 4. A study of the Young’s modulus (Modulus of Elasticity) for different materials for the fabrication of fixed prostheses using resonance frequency analysis and comparative characteristics with the elasticity of the mandibular bone. 4.1. A study of the Young’s modulus of metals; 4.2. A study of the Young’s modulus of ZnO2; 4.3. A study of the Young’s modulus of PMMA; 4.4. A study of the Young’s modulus of PEEK; 4.5. A study of the Young’s modulus of kevlar; 4.6. A comparative characteristics of the Young’s modulus of the materials in subtasks 4.1. to 4.5. and the Young’s modulus of the mandibular bone (in literature); 5. A retrospective study of 76 clinical cases of edentulous lower jaw patients treated with fixed implant-retained prostheses for a period of 12 years; 5.1. Presence or absence of mechanical problems; 5.2. Presence or absence of bone loss; 5.3. Presence or absence of functional discomfort; 6. A survey for the design and the materials used in the fixed implant-retained prostheses treatment of edentulous mandible; 6.1. A survey with dentists; 6.2. A survey with dental technicians; 6.3. A survey with patients. Scientific contributions: 1. For the first time in Bulgaria a study on the mandibular flexure was performed. 2. The dependence of the mandibular flexure on the degree of bone loss has been proven. 3. The decrease in flexure in edentulous mandible decreases in medial direction has been proven. 4. For the first time in Bulgaria a study on the relevance of the bone substitute material in bone grafting procedures on edentulous mandible for the mandible flexure was conducted. 5. The reason for peri-implant bone loss around the most distal implants in the treatment of edentulous mandible with fixed implant-retained prostheses has been confirmed. 6. A comparative study on Young’s modulus of edentulous mandible and the materials used for the fabrication of fixed prostheses was performed. 7. For the first time in Bulgaria social media was used for collecting information on a specific issue in the field of prosthetic dental medicine. Applied scientific contributions: 1. For the first time in Bulgaria information about dentists’ and dental technicians’ professional preferences for implant treatment with fixed prostheses was collected and processed. 2. The need for taking the final impression for fixed implant-retained prostheses in physiological rest position. 3. An optimized protocol for determining the number of implants in the treatment of edentulous patients with fixed implant-retained prostheses was created. 4. An optimized protocol for determining the position of implants in the treatment of edentulous patients with fixed implant-retained prostheses was created. 5. The need of using materials with Young’s modulus close to the bone’s one in the treatment of edentulous patients with fixed implant-retained prostheses was confirmed. 6. The need of separating the fixed implant-retained prostheses in the treatment of edentulous patients was confirmed. 7. The advantage of using removable implant-retained prostheses in the treatment of edentulous lower jaw was confirmed. 8. It was confirmed that in the treatment of edentulous lower jaw with fixed implant-retained prostheses, the choice of a reduced number of implants and shorter dental arch create less premises for peri-implant bone resorption in the area of the most distal implants.
