The nature, extent, and range of evidence of the premature constriction or closure of the ductus arteriosus in utero unrelated to known non-steroidal-anti-inflammatory drug exposure: a scoping review

Abstract

dc:description.abstract

Background: During pregnancy, the ductus arteriosus allows blood to bypass the fetal lungs. Non-steroidal anti-inflammatory drug (NSAID) use in late pregnancy can cause premature constriction or closure of the ductus arteriosus (PCDA), though cases also occur without NSAID exposure. Understanding non-NSAID-related PCDA (NNPCDA) is important for prenatal care. Objective: To synthesize current knowledge about PCDA with no known NSAID use. Method: A scoping review was conducted using established frameworks. Published and grey literature were systematically searched, screened by three reviewers, and synthesized. Results: The scoping review identified 179 NNPCDA sources. Many discuss potential PCDA risk factors, including non-NSAID medications and polyphenol-rich foods. Conclusion: The literature on NNPCDA is dominated by case reports, with limited empirical evidence in both humans and animals. Most exposures are reported in few studies without further investigation. Aside from polyphenol-rich foods, evidence on other risk factors remains sparse, highlighting the need for more comprehensive research.
