Tailoring health message frames to motivational orientation: A scoping review

BACKGROUND: Health behaviors are important determinants of health outcomes, but health behavior change interventions and communications remain limited in effectiveness. Tailoring communications to individual differences may be promising. One such individual difference is motivational orientation. This scoping review aimed to describe the nature and extent of research analyzing the moderating role of motivational orientation (described by Reinforcement Sensitivity Theory) in the relative effectiveness of gain- and loss-framed health messages. METHODS: Studies were included in the scoping review if they were systematic reviews or experimental studies in which the message frame was manipulated, the outcome variable was related to health behavior, and motivational orientation was measured as a possible predictor. Databases (Academic Search Ultimate, PsycInfo, PubMed), search engines (Google Scholar), and citation snowballing were used to identify studies. Study characteristics were extracted from each article selected for inclusion. RESULTS: A total of 20 experimental publications and 1 systematic review were selected for inclusion. Most studies provided moderate to strong support for matching message framing to motivational orientation to improve message effectiveness, but there was considerable variation in the terminology used by the studies. DISCUSSION: This scoping review, while not exhaustive, provides an overview of this heterogenous body of work. Efforts to synthesize findings through systematic reviews, meta-analyses, and the development of an overarching framework may help advance scholarly efforts in this field.
