Effet des statines sur les maladies cardiovasculaires chez les personnes âgées de 75 ans et plus

Introduction: As the population ages the rate of cardiovascular events is continually increasing. Compared to younger, people aged 75 and over are three times more susceptible to cardiovascular complications. According to Canadian guidelines statins are recommended for the prevention of cardiovascular diseases. However, the effects of statins in preventing cardiovascular events in people aged 75 and over remain unclear. In addition, this population is often absent or underrepresented in most studies. Objective: Our research aim was to study the association between the use of statins and the occurrence of angina, myocardial infarction and stroke in people aged 75 years and older. Methods: In this retrospective cohort study, we used the database of the CIRESSS (Computerized Center for Evaluative Research in Health Services and Care) at the University Hospital of Sherbrooke (Quebec, Canada) (CHUS). 4089 men and women aged 75 and over, admitted to the Sherbrooke University Hospital and recorded in the electronic medical records (Ariane) between 2007 and 2017, were classified into two groups: those who used the statin (GS) and those who did not (GNS). The effect of statin use on the occurrence of myocardial infarction, angina or stroke was analyzed using logistic regression. Results: 143 angina incidents were recorded in the GS group compared to 53 events in the GNS group (OR 3.37 ; 2.26-5.07 ; p <0.001). There were 435 myocardial infarction occurrences in the GS group compared to 157 in the GNS group (OR 4.43 ; 3.44-5.71 ; p <0.001). Regarding stroke, it was higher in the GS group at 9.32% compared to 5.93% in the GNS group with no signification (OR 0.74 ; 0.53-1.02 ; p=0.0676). Conclusion: The use of statins was significantly associated with the increased risk for coronary heart disease. However, no significant effect was observed on the stroke.
