Σχέση σιδήρου και φερριτίνης ορού μητέρας και νεογνού

Iron status, including haemoglobin, S-iron and S-ferritin was evaluated in 256 healthy mothers at parturition and in 256 of their term newborn infants. Mothers were divided in two groups. Group A included 188 mothers iron-supplemented and group Β 68 mothers non iron-supplemented in their pregnancy. Of the group A 19.1% had a S-ferritin level <12μg/l and Hb level <11 g/dl, 6.3%. Of the group B 82.3% had a S-ferritin level <12 μg/l and Hb level g/dl 23.5%. Newborn infants had higher Hb, iron and ferritin than mothers in the two groups. Group A: median Hb 15.5 g/dl versus 13.3 g/dl (p<0.001) median iron 98.7 μg/dl versus 92.6 μg/dl (p<0.05) and S-ferritin 156.6 μg/l versus 26.4 μg/l (p<0.001). Group B: median Hb 15.1 g/dl versus 11.6 g/dl (p<0.001) median iron 84.7 μg/dl versus 82.4 μg/dl (p>0.05) and S-ferritin 106.6 μg/l versus 7.7 μg/l (p<0.001). Mothers of group A had higher Hb level than mothers of group B (median 13.3 g/dl versus 11.6 g/dl, p<0.001) higher iron level (median 92.6 μg/dl versus 82.4 μg/dl, p<0.01) and higher S-ferritin level (media 26.4 μg/l versus 7.7 μg/l, p<0.001). The same difference appeared in the infant of group A versus newborn of group B: higher Hb level (median 15.5 g/dl versus 15.1 g/dl, p<0.05) higher iron level (median 98.7 μg/dl versus 84.7 μg/dl, p<0.01) and higher S-ferritin level (median 156.6 μg/l versus 106.6 μg/l, p<0.001). Iron levels in group A, r=0.39, p<0.01 in group B). S-ferritin appeared to be the best single indicator of maternal iron status. Ferritin levels in newborn infants were correlated to levels in mothers only in the group B (r=-0.31, p<0.05), indicating that fetal iron reserves are dependent on maternal iron stores only if their stores are depleted. The maternal ferritin appeared to be lower in mothers who had three or more previous pregnancies or were socially deprived.
