Αυτοάνοσες εκδηλώσεις και μονοκλωνική εξεργασία στη Β λεμφοκυτταρική νεοπλασία

The aim of this study was to investigate the monoclonal or oligoclonal process and autoimmune features of Non-Hodgkin Lymphomas (NHL). The study included 38 patients suffering from NHL and CLL of different disease stages and grades of malignancy. In 72% of the cases of NHL and 31% of the cases of CLL studied, monoclonal or oligoclonal immunoglobulins were detected in the sera. The presence of monoclonal immunoglobulins did not correlate with disease stage or grade of malignancy. It did although correlate with disease remissions or exacerbations indicating that such immunoglobulins could be useful tumor markers. Autoimmune parameters in NHL were not found to be significantly different from these of patients with solid tumors or the normal controls. For this study, a polyclonal antiidiotype was prepared against a monoclonal IgMk rheumatoid factor which was isolated from the type II cryoglobulin of the patient with Sjogren's syndrome. The presence of this idiotype was investigated in the sera of patients with NHL, Waldenstrom's macroglobulinemia and a number of autoimmune diseases. Two out of six patients with Waldenstroms macroglobulinemia were found positive for the presence of the idiotype, whilst all patients with NHL were negative. The idiotype was also found in 62.5% of Sjogren's syndrome and 27% of rheumatoid arthritis patients. No patient with systemic lupus erythematosus neither any normal control were found positive. The presence of the idiotype in Sjogren's syndrome correlated with the monoclonal process and the systemic manifestations of the disease. In conclusion, B-lymphocytes which bear cross-reactive idiotypes could possibly play a role in B-cell neoplasia.
