Prognostic Value of Bone Marrow Micrometastasis Detected by Nnested RT-PCR for Cytokeratin 19 and Mammaglobin in Breast Cancer

BACKGROUND/AIMS: Breast cancers in the early phase frequently undergo distant metastasis, and survival of patients is greatly dependent on distant metastasis. Occurrence of micrometastasis has been suggested to relate with prognostic features of breast cancer, such as lymph node metastasis and the presence of vascular invasion. The aim of this study was to examine the presence of keratin-19 and mammaglobin mRNA of epithelial tumors in bone marrow aspirates obtained from breast cancer patients and its possible correlation with tumor staging and disease free survival. METHODS: Bone marrow samples were obtained from 250 breast cancer patients at the time of surgery. We separated the mononuclear fraction from the samples and carried out nested reverse transcriptase polymerase chain reaction (RT-PCR) for the detection of keratin-19 and mammaglobin mRNA with two different pairs of primer. After operation, the patients were followed up by 3-month intervals. We studied possible correlation with tumor size, nodal involvement, stage, and distant metastasis. RESULTS: Seventy-five out of 250samples were studied cytokeratin 19 and others were studied cytokeratin and mammaglobin. Median follow-up time was 21.1 months. Sixty-five(26%) out of 250 samples were cytokeratin 19 positive and 25 (14.3%) out of 175 were mammaglobin positive. Eight cases (12.3%) in cytokeratin positive group showed recurrent legions in the distant organ. Whereas six (3.2%) out of 185 cytokeratin negative patients had distant recurrences. Mammaglobin was not correlated with distant recurrences. Bone marrow micrometastasis, stage, axillary nodal status, and estrogen receptor were independent. CONCLUSIONS: Bone marrow micrometastasis detected by nested RT-PCR for cytokeratin 19 can be a useful predictive marker for distant recurrence of breast cancer.
