A Clinical Characteristics of Adverse Drug Reaction in Pediatrics

PURPOSE: The drugs used for prevention of disease, medical treatment and research sometimes show the undesired adverse drug reactions. These unexpected adverse drug reactions are typically not related with the used capacity nor with pharmacological mechanism of individual drug. Thus, attention should be paid because the symptoms require first aid. METHOD: Of the infant patient who were outpatients or hospitalized in the department of Pediatrics of Ajou University Hospital for 10 years from January, 2000 to December, 2009. 99 patients who had shown adverse drug reactions were selected as subjects. And drug challenge test was performed for 16 patients to find out the drug cause and observed the result by age, gender, past illness, underlying disease, symptom and estimated drug to analyze. RESULT: It is estimated that the drug causes of 99 patients who had adversed drug reactions were antibiotics of penicillin type (18 patients, 18%), Cephalosporin (7 patients, 7%), Macrolides (3 patients, 3%), NSAID (13 patients, 13%), Acetaminophen (8 patients, 8%), Phenytoin (12 patients, 12%), Anti-cancer medicine (4 patients, 4%), Sedative (1 patient, 1%) and Antiflammatory (1 patient, 1%). For 16 patients among them, drug challenge test was performed. As a result, the drugs that have shown positive reaction were NSAID (6 patients, 38%), Acetaminophen (5 patients, 31%), Penicillin (4 patients, 25%), Cephalosporin (3 patients, 19%), Cotrimoxazole (1 patient, 6%), Macrolide (1 patient, 6%) and Lactose (1 patient, 6%). CONCLUSION: In this study, we evaluated the common causes, severities, and clinical manifestations of ADRs in 99 Korean infants and children. And successfully performed oral drug provocation test in 16 patients. These result could provide an important guidance to diagnose and managing ADRs in Korea infants and children.
