Beta2-microglobulin and antichromatin antibodies can be a disease activity markers in Systemic Lupus Erythematosus in Koreans

dc:descriptionBACKGROUNDS: Beta2-microglobulin and antichromatin antibodies have been described in patients with systemic lupus erythematosus (SLE) and it has been suggested that their presence is associated with lupus disease activity. We have investigated the clinical significance of beta2-microglobulin and antichromatin antibodies in Korean SLE patients. MATERIALS AND METHODS: Blood samples were collected from SLE patients (n=100), rheumatoid arthritis (RA) patients (n=50) as disease controls, and normal healthy controls (NC, n=50) at the Ajou university hospital. All of patients fulfilled the ACR classification criteria for SLE and RA, respectively. The presence of beta2-microglobulin and antichromatin antibodies was investigated by enzyme linked immunosorbent assay (ELISA). The levels of beta-2 microglobulin and antichromatin antibodies were expressed as arbitrary unit (AU). The SLE patients were also evaluated for clinical, laboratory parameters and systemic lupus erythematosus disease activity index (SLEDAI). RESULTS: The beta2-microglobulin level of the SLE group (2.64 ± 0.11 AU) was higher than the RA group (2.14 ± 0.10 AU, p value<0.001) and NC (2.14 ± 0.04 AU, p value < 0.001). The antichromatin antibody level of the SLE group (118.41 ± 41.87 AU) was higher than the RA group (49.70 ± 20.73 AU, p value<0.001) and NC (33.82 ± 24.20 AU, p value < 0.001). The antichromatin antibody was frequently associated with SLE with high sensitivity and specificity. The beta2-microglobulin levels were significant higher in SLE patients with serositis, oral ulcer, and lupus nephritis. The antichromatin antibody levels were significant higher in SLE patients with arthritis. A significant correlation was found between the levels of beta2-microglobulin and each of anti-dsDNA antibody (r=0.305, p=0.003), anemia (r=-0.433, p<0.001), complement (r=-0.231, p=0.032) and SLEDAI (r=0.460, p<0.001). A significant correlation was found between the levels of antichromatin antibodies and each of anti-dsDNA antibody (r=0.474, p<0.001), leucopenia (r=-0.268, p=0.008), complement (r=-0.228, p=0.033) and SLEDAI (r=0.278, p=0.006). CONCLUSIONS: These data suggest that the measurement of beta2-microglobulin and antichromatin antibodies appear to be a useful addition to the laboratory tests that can help in the diagnosis and treatment of SLE, and can be a useful biomarker.
