Natural History and Affecting Factors on Progression of Prehypertension and Prediabetes

dc:descriptionPURPOSE: The purpose of this study was to investigate the natural history of prehypertension (PreHTN) and prediabetes (PreDM) and to determine the effect of changes of cardiovascular disease (CVD) risk factors on progression from PreHTN and PreDM to hypertension (HTN) and diabetes mellitus (DM) through a 8-year prospective KCPS (Korean Cancer Prevention Study) data from the National Health Insurance Corporation in Korea. SUBJECTS AND METHODS: A total of 49,228 individuals, aged 30 to 54, without HTN at baseline (1994-1996) were followed up to 2004 for PreHTN study. For PreDM study, a total of 138,732 individuals, satisfying the inclusion criteria, were included. All subjects had undergone biennial health examination including biochemical measurements and health behaviors. RESULTS: The 8-year cumulative incidence rate of HTN in those with optimal, normal, and high-normal blood pressure (BP) was 175, 386 and 615 per 1,000 person-year respectively. In the Cox proportional hazard model, changes of CVD risk factors such as abstinence from drink, participating in a regular exercise, and reducing body mass index (BMI) level, and controlling total cholesterol (TC) level played a protective role on progression to HTN in both men and women. The 8-year cumulative incidence rate of DM in those with normal fasting serum glucose (FSG), stage 1 IFG (impaired fasting glucose), and stage 2 IFG was 37, 146, and 329 per 1,000 person-year, respectively. In the Cox proportional hazard model, changes of CVD risk factors such as smoking cessation, abstinence from drink, involving in a regular exercise, controlling BP, and lowering TC level diminished the progression to DM in prediabetic population. CONCLUSIONS: Positive changes of CVD related behaviors diminished the progression rate of HTN and DM. This study suggests that individuals with PreHTN and PreDM should be targeted for specific health behavioral intervention to prevent the progression of HTN and DM. In addition, large intervention studies should be establish ungently in order to prove results of this study.
