Die Beurteilung von Veränderungen der myokardialen Perfusion nach PTCA bei akutem Myokardinfarkt durch wiederholte Bestimmung des Myocardial Blush Grade

dc:descriptionThis study focused on the evolution of myocardial perfusion during follow-up after primary angioplasty for acute myocardial infarction (AMI) and its association with final left ventricular (LV) function. The angiographic myocardial blush grade (MBG) was analyzed immediately after intervention and at a follow-up check (7.5 ± 5.6 months later) in a sample of 101 patients with a first-time AMI. Further parameters included Ckmax, sum of ST-segment elevation, TIMI frame count and LV ejection fraction. The myocardial perfusion (MBG) at follow up was persistently abnormal in 19 patients (20%), improved from previously abnormal to normal in 30 patients (31%), remained normal in 40 patients (42%), and deteriorated from normal to abnormal in 7 patients (7%). Comparing the evolution groups it was ound that patients with an improved MBG had a better LV ejection fraction at follow-up than thoses patients with persistelntly abnormal blush. Moreover, the factor evolution of MBG had a better predictive value for LV ejection fraction at follow-up than acute MBG only. Regression analysis proved that the evolution of MBG from AMI to follow-up represents an independent predictor of LV function in addition to the initial size of jeopardized myocardium as defined by the sum of ST-segment elevation and infarct location. The results of the study show, that the myocardial perfusion after AMI is characterized by continous changes, as indicated by repeated MBG analysis. The improvement of tissue reperfusion turned out to be associated with infarct size
