Bedeutung der Neoangiogenese für die Prognose des Prostatakarzinoms : ein immunhistochemischer Ansatz unter Verwendung des Endothelmarkers CD 31

Introduction: Neoangiogenesis is a vital component of tumor growth and is supposed to correlate with the malignant behaviour of carcinoma of the prostate (CaP). As Gleason's grade, Gleason's score, tumor volume, tumor-stage and PSA are established prognostic indicators in CaP we checked the hypothesis that the mean increase in number of blood vessels as their decreasing radius as an indicator for tumor related vessel growth could be correlated with the established parameters. Methods: Archival tissue of 40 hormonally untreated patients undergoing radical prostatectomy for CaP were studied by immunostaining for CD 31 related antigen. By using a computerized image analysis system the micovessel density (mvd) and the radius of the vessels were evaluated in twenty fields of 0,25 mm^2 in the two areas of the tumor responsible for the grading according to Gleason. A non tumor area was used for internal control. Spearman und Pearson correlation and linear regression analysis were used for statistics. Results: For the cohort a correlation of the impairement of the prognosis indicated by the established parameters with an increase of mvd and/or a decrease in radius could not be found. Comparing the data for the individual it could be revealed that different tumor areas with according histological differentiation showed no consistent mvd or mean vessel radius. Conclusions: In this study the neoangiogenetic status represented by mvd and mean vessel radius was independent from the established prognostic parameters of the carcinoma of the prostate. Several conclusions are thinkable: 1 Neoangiogenesis does not reveal any information about the prognosis of the disease. 2 Mvd and blood vessel radius do not reveal the neoangiogenetic status of the tumor. 3. Mvd and blood vessel radius are two putative independent predictors in localized CaP.
