Fluoreszein-Angiographie und Progression des Glaukoms

dc:descriptionGlaucoma is a chronic progressive disease and one of the most frequent causes leading to blindness in the western world, characterized by damage of the optic nerve and visual field loss. Various theories have been proposed to explain the pathogenesis of glaucoma. The pathogenetic concepts may be divided in a mechanical, pressure-related and a vascular approach. In the vascular theory a low perfusion of the optic nerve is described, if primary or secondary due to the nerve fiber loss is still discussed. In addition to the individually increased intraocular pressure, other risk factors for the development (age, myopia, black race and systemic vascular risk factors) and for the progression (age and morphological changes like exfoliation syndrome and disc haemorrhages) of glaucoma have been described. In this retrospective clinical study, patients with normal tension glaucoma, primary open- angle glaucoma and ocular hypertension were included. The association between different parameters of perfusion and the progression of the visual field loss was analyzed. Using fluorescein angiography of the optic disc and retina with a scanning laser ophthalmoscope and digital imaging analysis, fluorescein filling defects of the optic nerve head and retinal arteriovenous passage time were measured. To evaluate the progression of glaucoma, visual fields were analyzed during a long-term follow-up. The visual field progression (i.e. mean deviation progression per year) was correlated to the fluorescein angiographic findings. In addition, the influence of known risk factors (age, intraocular pressure and the extent of the visual field damage) on the progression of glaucomatous visual field loss was analyzed using a multivariate approach. Progression of glaucomatous visual field loss was associated with increased arteriovenous passage times in normal tension glaucoma. In patients with primary open-angle glaucoma, a significant correlation between visual field progression and intraocular pressure was found. Additionally, in patients with primary open-angle glaucoma and normal tension glaucoma a correlation of progression and age was observed. These results demonstrate that the decreased ocular blood flow may play an important role in the pathogenesis and progression especially in normal tension glaucoma. The impact of the vascular risk factors for the progression of glaucoma is again emphasized by this study.
