Implantierbares elektromechanisches Blutpumpensystem zur langfristigen partiellen Herzentlastung

dc:descriptionNowadays, Ventricular Assist Devices (VADs) have mostly been employed as bridge to transplantation. A new field of application for VADs represents the support of the failing heart function with the aim of heart recovery, "bridge to recovery". This dissertation describes the development of a long-term implantable therapeutic heart support system for the recovery of heart muscle by partial unloading of the left ventricle. The development of the pump unit is the main target of this work. Due to size minimization of the pump unit, a new gear type is developed which converts a continuous, unidirectional rotation of the electric motor into a translatory pusher plate movement (patent No. DE 197 26 702,5 1997). This so called "Kaufmann-Förster" (KF) gear unit is characterized regarding the pumping characteristics. Result of this construction process is a totally implantable flat displacement blood pump with a titanium alloy housing and a polyurethane pumping chamber, called VERSUS pump (Ventricular Recovery Support System). The pump characteristics with pump flow rates and the necessary power consumption are investigated in the theoretical analysis. The overall efficiency of the pump unit, the hydraulic performance and the damage of the red blood cells caused by the pumping mechanism are subject of the experimental in vitro investigation. A physiciologically adequate discharge by the VERSUS blood pump was proven in three animal studies. The reliability of the drive system, the good handling of the pump during operation and the good anatomical fitting of the pump during implantation in the upper thorax are successfully proven.
