Die Endoprothetik des oberen Sprunggelenkes : Metaanalyse der Sprunggelenkendoprothetik unter Anwendung eines Evaluations-Scores

BACKGROUND: Following the failures of the first and the second generation of ankle joint endoprostheses, the newly developed third generation won increasing acceptance. Although there is a pronounced variance of the concepts in the literature search, so far no comparison for the fixation of these implants of the third generation has been conducted.MATERIAL AND METHODS: In the literature search available, results and complication rates of the implants of the third generation are compared. In order to make an exact analysis possible, a Complications-Revisions-Score was developed. This evaluation system focuses on the number of the complications and revisions in the post-operative process. The different follow-up examination times enter evaluation too. The studies with the selected endoprosthesis designs were subsequently analyzed with the help of the Complications-Revisions-Scores and received appropriate points of evaluation. Depending on the number of points obtained under the endoprosthesis designs, a ranking was provided based on the Complications-Revisions-Score.RESULTS: The literature search consisted of various databases such as PubMed and included the ankle joint endoprostheses of the third generation. Of a total of 53 publications concerning ankle-joint-Prosthetics, 26 studies dealt with the endoprostheses of the third generation. Of these, 23 studies with 8 different endoprostheses designs were included. After the application of the Complications-Revisions-Scores, the following implants showed small complication rates: New Jersey endoprosthesis (3.11 points), Buechel Pappas endoprosthesis (3.84 points) and the Salto endoprosthesis (4.31 points).CONCLUSION: The New Jersey, Buechel Pappas and the Salto endoprosthesis exhibited the smallest complication rates. The complication and revision rates appear to be adequately reflected in the newly developed score.
