Due to the frequent ventricular fibrillation as initial cardiac rhythm in out-of-hospital cardiac arrest, defibrillation is essential. For several years, Automated External Defibrillators (AED) are available which enable defibrillation by other groups than physicians and healthcare professionals. In order to intend the need and a possible selection of ideal locations of AED particularly for the district Düren, altogether 560 out-of-hospital courses of resuscitation from the period of 4 Years were retrospectively analyzed. In the course of the 560 resuscitations in 44% of cases, a defibrillator was used. Only 20% of the cases were detected while an initial ventricular fibrillation. The outcome of these patients could be increased by the use of the defibrilator on 17%. Only professional rescue squad and an established physician have served the tool in the examined period. As among other things the rescue personnel thereby only in altogether 37% of cases within in the auxiliary period of five minutes at the location, recommended by the ERC, the need for a early defibrillation is quite given also by First Aiders respectively laymen. The survival rate of the patients with initial ventricular fibrillations lies in the district Düren with 17% in the international comparison in the lower range. Hope to obtain with a started PAD program a large progress in the fight against sudden cardiac death, the numbers in the district Düren and the data situation in the literature do not seem to be very promising. Finally, several limitations of this concept exist, such as low frequency of sudden cardiac death in public (4% vs. 73% in the home environment), the previously rare use of publicly placed AED, the low cost-effectiveness and the critical legal situation. Due to these restrictions other measures are more promising than the efforts to spread AED ubiquitously. These measures include the proper observance of the rescue chain, the implementation of AED-equipped first-responder teams, and in particular a better education for the population in the basic measures of cardiopulmonary resuscitation. It pointed out by the analysis of the courses of resuscitation, that the average probability of survival of the resuscitations (dismissal from the hospital) in the district Düren (9%) could be substantially improved under the following conditions. The resuscitation of the laymen have a vital importance. If has been demonstrated in a cardiac arrest a lay resuscitation increases the outcome of patients already at 17%. If the time of the collapse was observed, the outcome results of 21%. And if finally thereby the responsible emergency service is still within three minutes locally, the survival rate rises even on 29%.
