N[Epsilon]-(Carboxymethyl)Lysin und 3-Nitrotyrosin im Atemkondensat von Patienten mit chronischem Nierenversagen

Exhaled breath condensate (EBC) is the exhaled water steam with dissolved substances, which are set free from the airway lining fluid in small droplets. EBC can be collected and analysed by cooling down the exhaled air and collecting the condensate. Oxidative stress is the imbalance of increased production of oxidative radicals and decreased antioxidative mechanisms and is associated with chronic renal failure, diabetes mellitus and asthma. Two markers of oxidative stress are carboxymethyllysine and nitrotyrosine. Both have been detected in EBC from patients with diseases, which are associated with oxidative stress. Increased concentrations of these markers have been detected in the serum of patients with chronic renal failure, but no one looked for the markers in the EBC until now. Aim of this study was the examination, if carboxymethyllysine and nitrotyrosine is increased in serum, urine and EBC of patients with chronic renal failure under the idea that the possible influence of chronic renal failure has to be considered by interpreting the results of EBC analysis. 27 patients with chronic renal failure undergoing ambulant haemodialysis therapy and 28 healthy people as control group have been examined. The dialysis patients had significant increased concentrations of CML- and creatinine than the control group. CML correlates positive with the creatinine and negative with the GFR. Influence of age, smoking and diabetes mellitus was not found in this study. In the urine the concentrations of CML and creatinine, as well as CML/creatinine did not differ significantly. A significant difference concerning the concentrations of CML/lysine in the EBC between the dialysis group and the control group was found, when the patients with diabetes mellitus were excluded. CML/lysine was significant lower in the EBC of diabetics with chronic renal failure than in the EBC of non-diabetics with chronic renal failure. There was no significant difference between the concentrations of nitrotyrosine and nitrotyrosine/tyrosine in both groups. There could not be found an influence by smoking or suppression of the immune system. Lysine, tyrosine and proline were detected in significant lower concentrations in the EBC of patients with chronic renal failure than in the healthy group. Smoking did not influence the results. All results were proved with statistic methods. To interpret the results of the analysis of EBC there has to be considered, that CML/lysine is increased by chronic renal failure. But it is important to differ between diabetics and non-diabetics with chronic renal failure. To assess the influence of diabetes on CML/lysine there have to be further studies. The concentrations of amino acids in the EBC of patients with chronic renal failure were obviously lower. There is no influence on the concentration of nitrotyrosine.
