The demand for revisions of hip joints prosthesis has been increasing with the rising life expectancy and the growing number of young patients, who need a hip replacement. As the success and the sustainability of a revised prosthesis is decisively dependent on sufficient bone material, bone preserving replacement systems have been gained in importance in the last years. Hence the surface hip replacement is to be pointed out, because only minimal bone resection is necessary in order to apply this joint prosthesis. Whereas metal surface replacements have been implanted serially in the last five to ten years, there has been no such ceramic prosthesis is available on the market. In this thesis an analysis of reliability for ceramic surface hip joint prosthesis is accomplished with the help of the finite element method. On the basis of a developed prosthesis-bone model the effects of different contact areas between prosthesis and bone at varying loading conditions are investigated. In the front of interest is the influence of the specific elected contact areas on the long term stability of the prosthesis compound and therewith the service life of the surface prosthesis. At the same time the stress of the bone is also taken into consideration. The aim of this thesis is to find out, if, where and why the ceramic and/or the bone meet an overstraining and how this can be avoided. In this way it is to be clarified, if the reliably realization of surface hip replacement made of ceramic is – analogical to the metallic example of the Birmingham Hip Resurfacing (BHR) – is possible. In summary it can be recorded that the present calculations prove a principal qualification of a thin walled ceramic prosthesis for surface hip replacement. Ceramic surface prosthesis down to a wall thickness of 3 mm exhibit no critical stress values respectively fracture probabilities at maximum possible transfer radii and adequate force flow - even at disadvantageous loading angles. Only for a press fit an overstraining of the bone is calculated.
