Vergleichende Beurteilung verschiedener echokardiographischer und elektrokardiographischer Parameter zur Vorhersage der Erholung der linksventrikulären Funktion bei Patienten mit akutem St-Hebungsinfarkt

Despite successful intervention and reopening of closed cardiac vessels in patients with acute myocardial infarction it can come to an irreversible damage to the microcirculation and endo-myocardial level due to the time latency. The extent of this so-called "no-reflow phenomenon" is combined with a loss of function, which affects Mortality and the prognosis of patients. A beside viable, non-invasive method of investigation is the myocardial contrast echocardiography which can show the remaining lack of the myocardial perfusion in addition to the representation of the morphology of the left ventricle at present. To a prospective clinical study 52 patients were included, who suffered an acute myocardial infarction and were then treated at the University Hospital Aachen by emergency angioplasty. Furher by resultant opacification scores evaluated by myocardial contrast echocardiography and remaining ST-elevations after angioplasty were regarded to the recovery of left ventricular function and the formation of a left ventricular remodeling. The identified perfusion-scores have shown that they can be applied to evaluate an individual patient prognosis regarding the extent of remaining restrictions of the ventricular function after months. A concrete statement, whether and to what extent a remodeling develops, could not be evaluated. Despite of increased likelihood every remaining lack of perfusion after infarction is not socialized with the development of a remodeling. This suggests that the development of a left ventricular remodeling can be understood as a multifactorial done and is not solely attributable to a limited perfusion. The study also shows that the only rudimentary ECG can not be used to show the exact extent of a non perfused area after intervention and to reveal parameters of left ventricular function and the development of a remodeling. Thus The ECG has still got its greatest significance to define infarct shape and the infarct location. In its prognostic significance, in terms of recovery of left ventricular function recovery after months and development of a remodeling after months, it is clearly restricted.
