Therapeutische Effekte von vier verschiedenen Behandlungsmethoden bei der Korrektur von Distalbisslagen unter besonderer Berücksichtigung des Weichteilprofils

The aim of this study was to determine and to compare the skeletal, dental and facial changes contributing to Class II correction in 96 adolescent and adult patients treated either with a combined orthodontic-orthognathic surgery approach, a Functional Mandibular Advancer, a Herbst appliance or with four premolar extraction. Effects on the soft tissue facial profile were of great interest. Lateral headfilms from before and after treatment were analyzed using self-defined cephalometrics. The actual treatment effects were determined by a control group, which was set up on the basis of the growth manual by BHATIA and LEIGHTON. The greatest improvement in sagittal occlusion was achieved in the surgery group by skeletal changes due to anterior displacement of the mandible. The greatest amount of skeletal and soft tissue facial profile convexity reduction was also acquired by surgery. The improvement in sagittal occlusion in the fixed functional appliance groups was achieved by dental more than skeletal changes. A reduction of profile convexity could also be shown. No forward development of the mandible could be found in the extraction group. The effects on the soft tissue profile are controversial. Considering the lip position no significant difference to the other three treatment methods could be determined. However extration therapy led to an increase of the nasolabial angle resulting in an undesirable esthetic outcome. The results of this study demonstrate the considerable amount of variability in forecasting soft tissue adaption in relation to the acquired changes in hard tissue. In addition to this it becomes apparent that all four mentioned Class II treatment methods may improve facial ethetics due to dentoalveolar effects regardless of skeletal changes.
