Childhood fractures are a particularly common paediatric problem. The data available to date unequivocally indicate that adipose tissue is an active participant in the regulation of bone homeostasis, and obesity is a new and recently recognised potential risk factor for disorders in bone strength. The current study includes a survey of the number and localisation of fractures suffered so far among 2513 schoolchildren in the city of Varna and a clinical part with a study of 41 obese girls. The overall fracture frequency found in our studied child-adolescent population of 13.1/1000 is relatively low compared to data from other European countries. In children with more than one fracture, as the number of fractures increased, there was an increase in the percentage of those who were overweight/obese, especially in girls. In severely obese girls, the amount of fat mass in the arm region exceeds that of muscle mass, which correlates strongly negatively with bone mineral content and bone area. This indicates that in overweight compensatory skeletal muscle development in the limb region is probably insufficient to build adequate underlying bone. In girls with high and extreme obesity, the whole-body bone area is found to be greatly reduced, as well as the bone area in the arms. The thesis research data are an excellent basis for future studies in the field of children's bone health in our country.
