Partielle Flüssigkeitsbeatmung im Vergleich zur konventionellen Beatmung : Auswirkungen auf Gasaustausch und Lungenpathologie im lungengesunden Neugeborenen-Tiermodell

Objective: To evaluate the effects of single (SLV), continuous partial liquid ventilation (PLV) and conventional ventilation on gas exchange and lung injury in a newborn healthy animal model. Design: Prospective, controlled, in vivo animal laboratory study. Setting: Research laboratory in a university setting Subjects: 19 newborn healthy male piglets (mean weight 1,9 kg, age 1-3 days) Interventions: The piglets were anesthetized, intubated und instrumented with vascular catheters. They were randomized in 3 groups: animals receiving 1 hour session of PLV followed by 23 h of CV (SLV), animals receiving continuous PLV over 24 hours (LV), conventionally ventilated animals over 24 hours (CV). The oxygenation index (OI), PaO2/FiO2 ratio, Ventilationindex (VEI) and pH were determined by beginning and hourly during the 24h course. After 24 hours the lungs were removed for histopathological examination. Measurements and main results: The CV group provided the best outcaome (p<0,05) with a continuously stable gas exchange over the 24h period. Between the LV and SLV groups no differences in PaO2/FiO2 ratio were observed however after 6h the OI and VEI deteriorated significantly in the SLV group (p< 0,04 and p< 0,01). CV animals demonstrated significantly higher signs of atelectasis in both upper (p<0,01)and lower (p<0,02) lobes compared to the liquid ventilated groups. A semi-quantitative lung injury score revealed most severe lung damage in the lower lobes of the SLV group compared to the upper lobes (p<0,01) and to the LV group (p<0,006). Conclusion: A prophylactic treatment with PLV led to an impaired gas exchange during the 24 h study. Continuous PLV provided though the best histopathologic outcome.
