Funktionelle Untersuchungen mit der transkraniellen Dopplersonographie bei Gesunden und Aphasikern

dc:descriptionBy means of transcranial doppler sonography blood flow changes of both middle cerebral arteries were measured simultaneously during cognitive stimulation with special consideration of the interhemispherical distribution, in order to evaluate functional recompensation mechanisms after cerebral damage. Test paradigms, which physiologically activate the left brain, lead in chronic aphasics to a predominantly right-hemispherical increase in blood flow velocity. This effect does not correlate with duration, strength or type of syndrome of aphasia; however a connection with rehabilitation success obtained under logopedic therapy exists. Acute aphasics show a fast restitution in the case of relative intactness of left-hemispherical structures (Karbe et al., 1998). In the case of more extensive damage of the left brain the recruiting barness of contralesional cortical areas indicates an easily more favorable prognosis. This lateralization of linguistic dominance seems to be promoted by a pre-existing bihemispheric linguistic network (Weiller et al., 1995), within those components language processing after cerebral infarction can be redistributed.
