Test paradigms, which physiologically activate the left brain, lead in chronic aphasics to a predominantly right-hemispherical increase in blood flow velocity. This effect does not correlate with duration, strength or type of syndrome of aphasia; however a connection with rehabilitation success obtained under logopedic therapy exists. Acute aphasics show a fast restitution in the case of relative intactness of left-hemispherical structures (Karbe et al., 1998). In the case of more extensive damage of the left brain the recruiting barness of contralesional cortical areas indicates an easily more favorable prognosis.
