This work seeks to assess this association in the largest cohort to date and become the first to assess MPV in injection drug use associated endocarditis cases (IDU-IE). Methods: Patient charts from 1/1/2004 - 9/31/2015 were manually reviewed. Only those who reported IDU in the 3 months prior to admission were classified as IDU - IE . IDU - IE cases were stratified into those that had injected oxymorphone (OE) and those who had not injected oxymorphone (OU) . Results: MPV is significantly associated with embolic disease of endocarditis even when demographics, clinical disease characteristics, and laboratory data are controlled for (OR 1.33 [ 1.07 - 1.64 ], p=0.002). Larger MPV at admission is associated with drug use ( p=0.0004 ) . Oxymorphone injection drug use was significantly more likely to be associated with MPV greater than or equal to the median of the reference range . ( 80.2% of the OE group vs 59.5% of the OU group, p= 0.048 ) . There is no evidence that dr ug use interacts with or moderates the relationship of MPV with endocarditis embolic disease.
