Abstract
dc:descriptionPeri-implant diseases are inflammatory diseases that affect the structures supporting dental implants. These diseases are often diagnosed as an advanced form of inflammation – peri-implantitis (PI) which leads to the loss of the dental implant. However, perimucositis (PM), which is the first to occur, could be easily cured. For this reason, tools for the early diagnosis of peri-implant diseases are needed. The development of such new diagnostic methods may be aided by the development of miRNAs, that regulate gene expression at the post-transcriptional level. As miRNAs have been found to be altered in expression in a number of diseases, including PI, these molecules, which are easily detectable in body fluids, have great potential to become non-invasive biomarkers of PI. The aim of this study was to evaluate miRNA expression profiles in peri-implant crevicular fluid (PICF) by NGS and RT-qPCR and to evaluate the diagnostic and prognostic potential of inflammatory miRNAs. The expression of miR−19a (P = 0,022) was significantly increased in the PICF of PI–affected participants. Moreover, advanced PI was distinguished from moderate PI by increased expression of miR–19a (P = 0.006) and decreased expression of miR–21 (P = 0.049). Moderate PI was distinguished from early-stage PI by increased expression of miR–19a (P <0.001) and –145 (P = 0.032). Also, miR–19a expression was significantly upregulated (P <0.001) while expression of miR–145 was downregulated (P = 0.032) in the group of moderate PI, as compared to the PI-unaffected group. The highest diagnostic potential was identified for combination of all seven miRNAs (AUC = 0.77; P <0.001), while prognostic potential for combination of miR–140–3p and –145 (AUC = 0.82; P <0.001 and AUC = 0.73; P = 0.015, respectively). In conclusion, the analyzed miRNAs are associated with the pathogenesis of peri-implant diseases and could potentially be used for the diagnosis and prognosis of peri-implant diseases. However, further studies are necessary to confirm the identified associations.
