The thesis includes theoretical and empirical research. The empirical part of the research was conducted in the Ruse region. It includes a quantitative study using an anonymous survey of 180 patients with type 2 diabetes mellitus; a quantitative study using a standardised interview of 70 nurses providing healthcare to patients with diabetes; and a quantitative study using an in-depth interview of 10 endocrinologists. For the experimental part, two research groups were formed: experimental and control, each with 15 patients. A comprehensive toolkit adapted to the characteristics of the studied population was developed and implemented: questionnaires, self-monitoring diaries, a health status monitoring card, and information materials. An author's program, \"I control diabetes\", was created and tested. Дисертационният труд включва теоретично и емпирично изследване. Емпиричното е проведено на територията на област Русе като: количествено проучване чрез анонимна анкетна карта на 180 (n=180) пациенти със захарен диабет тип 2 на възраст над 40 години; количествено изследване чрез стандартизирано интервю на 70 медицински сестри (n=70), предоставящи здравни грижи на пациенти с диабет; количествено изследване чрез дълбочинно интервю на 10 лекари-ендокринолози (n=10). За целите на експерименталната част са формирани две изследователски групи: експериментална и контролна, всяка с по 15 пациенти. Разработен и приложен е цялостен инструментариум, адаптиран към особеностите на изследваната популация: анкетни карти, дневници за самонаблюдение, карта за наблюдение на здравния статус и информационни материали. Създадена и апробирана е авторска програма „Аз контролирам диабета“.
