The study aims to investigate the expression of these five microRNAs in the plasma of patients with MDS and healthy controls, and to analyse their correlation with clinical, laboratory, and prognostic parameters, including R-IPSS risk groups and bone marrow blast percentage. The study is prospective and includes 40 patients with confirmed MDS and 10 healthy controls. MicroRNA expression was analysed using Real-Time PCR, and statistical analysis was performed. The results show significantly lower levels of miR-16, miR-144, miR-451a, and let-7a in MDS patients, with miR-451a demonstrating the highest diagnostic value. MiR-144 and miR-451a correlate with cytogenetic risk, while let-7a is associated with blast percentage and R-IPSS. LASSO analysis identified miR-22 and miR-451a as the strongest predictors for distinguishing patients from healthy controls. Correlation analysis revealed associations between the microRNAs and biochemical markers such as LDH and ferritin. Целта на проучването е да се изследва експресията на посочените пет микроРНК-и в плазмата на пациенти с МДС и здрави контроли, както и да се анализира тяхната връзка с клинични, лабораторни и прогностични показатели, включително рисковите групи според R-IPSS и процентът на бласти в костен мозък. Проучването е проспективно и включва 40 пациенти с потвърден МДС и 10 здрави контроли. Експресията на микроРНКите е анализирана чрез Real-Time PCR и е осъществена статистическа обработка на данните. Резултатите показват значително по-ниски нива на миР-16, миР-144, миР-451а и let-7a при пациенти с МДС, като миР-451а демонстрира най-висока диагностична стойност. МиР-144 и миР-451а корелират с цитогенетичния риск, а let-7a – с процента бласти и риска определен по R-IPSS. Чрез LASSO анализ са идентифицирани миР-22 и миР-451а като най-силни предиктори за разграничаване между пациенти и здрави контроли.
