The clinical appearance and therapeutic behaviour of individuals with ACS and concomitant COPD compared to a control group with no data on pulmonary obstruction were examined. The reasons for rehospitalisation, as well as the quality of life in both groups of patients, were investigated. The clinical and demographic characteristics of the deceased patients were analysed. Patients with ACS and COPD had a more prominent risk profile for ICD than patients with ACS and no COPD, according to the comparison analysis between the two study groups. Up to 12 months following the acute coronary event, people with pulmonary obstruction had an increased likelihood of being admitted to the hospital for cardiovascular disease. The most frequent cause was CHF, which was followed by ICD. In this patient sample, higher baseline CRP values, smoking, a higher BMI, and less physical exercise have been associated with higher mortality. The following scientific data demonstrates that people with ACS and COPD are a high-risk population that needs additional research and monitoring. Creating an exceptional algorithm is just one step towards improving the treatment these patients get.","Дисертационният труд представлява първото научно изследване върху рисковата, клинична характеристика и придържането към назначената медикаментозна терапия на пациенти с остър коронарен синдром (ОКС) и хронична обструктивна белодробна болест (ХОББ) за региона на Североизточна България. Разгледани са клиничната изява и терапевтичното поведение при лица с ОКС и придружаваща ХОББ, сравнени с контролна група без данни за белодробна обструкция. Проследени са причините за повторна хоспитализация, както и качеството на живот в двете групи болни. Анализирана е клиничната и демографската характеристика на починалите пациенти. Представеният сравнителен анализ между двете изследвани групи доказва, че пациентите с ОКС и ХОББ са с по-изразен рисков профил за ИБС в сравнение с болните с ОКС без ХОББ. Лицата с белодробна обструкция са изложени на по-голям риск от хоспитализация по повод ССЗ до дванадесети месец след острото коронарно събитие. Като най-честа причина се явява СН, следвана от ИБС. По-високият ИТМ, тютюнопушенето, по-ниската физическа активност и по-високите изходни стойности на CRP се асоциират с по-висока смъртност при тази кохорта пациенти. Получените научни данни потвърждават, че лицата с ОКС и ХОББ са високорискова група, която има нужда от по-задълбочено проучване и проследяване.
