An automated HB&L laser nephelometry system, Alifax, was used for rapid 4-hour screening of 1600 urine samples. For the first time in Bulgaria, a parallel study of residual antibiotic activity was performed to improve the clinical interpretation of the test. Patients were recruited based on clinical symptomatology, chemical examination of urine and sediment, and a completed medical questionnaire. Antibiotic susceptibility was determined by 3 methods: standard culture method according to Bauer-Kirby, automated HB&L system and automated VITEK 2 system. An analysis of the errors obtained by the automated system was performed in a short time (3-5 hours) against the culture 24-hour reference method. Additionally, the susceptibility to three antibiotic agents used in outpatient practice was studied: fosfomycin, nitrofurantoin, and 5-nitroxoline. A high concordance of results was demonstrated. The system showed high specificity, sensitivity, and positive and negative predictive values. Based on the results, a working algorithm for the accurate and rapid diagnosis of urinary tract infections on the day of puncture was developed. За целта е използвана автоматизирана система за лазерна нефелометрия HB&L, Alifax за бърз 4 часов скрининг на 1600 проби урина. За първи път в България е осъществено паралелно изследване на остатъчна антибиотична активност, за да се подобри клиничната интерпретация на теста. Пациентите са набирани на база клинична симптоматика, химично изследване на урина и седимент, попълнена медицинска анкета. Антибиотичната чувствителност е определена по 3 методики: стандартен културелен метод по Bauer – Kirby, автоматизирана система HB&L и автоматизирана система VITEK 2. Осъществен е анализ на получените грешки на автоматизираната система за кратко време (3-5 часа) спрямо културелния 24 часов референтен метод. Допълнително е проучена чувствителността към три антибиотични препарата, използвани в амбулаторната практика: fosfomycin, nitrofurantoin, 5-nitroxoline. Доказва се висока конкордантност на резултатите. Системата показва висока специфичност, чувствителност, положителна и отрицателна предиктивна стойност. Базирайки се на получените резултати е разработен работен алгоритъм за точна и бърза диагностика на уроинфекциите в деня на пободването.
