Controlled Arterial Hypotension in Shoulder Surgery // Контролирана артериална хипотензия при раменна хирургия

Controlled hypotension, namely the targeted lowering of systemic arterial pressure, has become established as a technique in various surgical procedures to reduce intraoperative blood loss and improve the visibility of the operative field. To achieve controlled hypotension, various methods and techniques – pharmacological and non-pharmacological, as well as a combination of these have been described in the literature. The search for an optimal solution remains in progress as an attempt to optimise surgical conditions while ensuring maximum patient safety. The "optimal" technique should provide a rapid reduction of arterial pressure and have a rapid effect while minimising negative effects on the organs. None of the methods for achieving controlled hypotension is universal or self-administrable. This thesis aims to highlight the advantages of controlled hypotension and the complexities of the technique. Through a meticulous review of the available literature, we aim to provide a comprehensive view of this technique and to expand the knowledge base regarding its safety and efficacy in surgical settings. In this study, we review the use of interscalene plexus block in combination with general anaesthesia to achieve controlled hypotension in surgical procedures of the shoulder joint and proximal humerus.
