Changes in Water-Electrolyte Balance in Organ Donors with Brain Death and Their Correction in the Intensive Care Unit // Промени във водно-електролитното равновесие при органен донор с мозъчна смърт и корекцията им в интензивната клиника
Abstract
dc:descriptionBrain death is a condition of complete and irreversible cessation of all functions of the brain and brainstem while maintaining circulation in the rest of the body. In approximately 80% of organ donors with brain death, damage to the posterior lobe of the pituitary gland is observed, leading to the development of diabetes insipidus with changes in water-electrolyte balance, hypovolemia, and circulatory instability. The lack of or improper control of this condition leads to hypovolemia, hyperosmolarity, hypernatremia, hypermagnesemia, hypokalemia, hypophosphatemia, and hypocalcemia. To correct these changes in water-electrolyte balance, the patient's therapy should include balanced saline solutions or those with low sodium content. In cases of persistent hypotension despite adequate volume replacement, compensating for the antidiuretic deficiency should be considered. This study aims to identify patients who have reached brain death and analyse their demographic and clinical indicators. The main objectives include analysing the changes in water-electrolyte balance by monitoring the dynamics in the values of Na+, K+, Cl-, Ca2+, and Mg2+, determining the types of medications used as part of the patient's therapy, and comparing global experience with our own.
