dc:description[EN] Correlations between diseases with metabolic disturbances and oral cavity state are carried out by means of various types of homeostasis (metabolic, immunological etc.), distortions of which lead to various diseases of the whole organism and the oral cavity as well. Diabetes mellitus (DM) is one of the most serious social significant diseases with a number of complications. It is a chronic disease with a life-long span. The term "diabetes" describes a group of diseases that are characterized by increased levels of blood sugar and disorders of carbohydrate, fat and protein metabolism. The correlation between a number of diseases and changes in the oral cavity in patients with DM is two way. On the one hand, hyperglycaemia leads to deterioration of oral hygiene and health and on the other - periodontitis is identified by many authors as a possible risk factor for poor metabolic control in patients with diabetes. Occurring oral manifestations in DM often lead to more frequent use of dental care. Aim of the study To make a complex oral diagnosis of children and adolescents with diabetes mellitus and children with obesity in order to establish the incidence and characteristics of oral pathology, the state of hard teeth tissue, periodontal and oral mucosa in order to develop a complex method for prevention and treatment. The study included 257 children, divided into 3 groups: children with diabetes mellitus - 114; children with obesity - 73 and 70 healthy controls in the same age range. Conclusions The study established that the most important risk factor in diabetic patients with a negative impact on their oral pathology is the bad glycaemic control (HbA1c - 8.9%) and frequent episodes of hyperglycaemia. There is a correlation between oral lesions and disorders in metabolic homeostasis. In children with diabetes mellitus and poor glycaemic control we found lesions in 14 patients (12.28%). In our patients with obesity risk factors for oral health risk factors were: short period of breastfeeding, early introduction of infant formula, dietary errors and lack of active fluoride intake. We found that children with diabetes have more often oral complications compared with children with obesity, which were significantly higher than in healthy controls (p <0.05). This requires early active prevention and treatment to improve glycaemic control and quality of life of patients. Contributions A complex oral diagnosis of children and adolescents with diabetes mellitus and those with obesity is performed for the first time in Bulgaria for establishing the frequency and characteristics of oral pathology, the state of hard teeth tissue, periodontal and oral mucosa. A correlation between oral lesions and disorders in metabolic homeostasis is established.
