Hypertension Care – Nurse’s Role in Ambulatory Monitoring /// Артериална хипертония – роля на медицинската сестра в амбулаторния контрол

The aim of the thesis is to interpret the role of the nurse in ambulatory control of hypertension via applying a Model for observation and control of the risk factors, keeping the prescribed medical therapy and improving the quality of life of the patients. Hypothesis: Applying the Model of observation of patients with poor blood pressure control, would lead to change in behaviour, an improvement of treatment would be achieved, and enhancement of the quality of life and the satisfaction of patients. The target of the test are patients with suboptimal blood pressure control, who have visited a specialized ambulatory practice in the town of Ruse - The specialized ambulatory outpatient practice (SAOP) of cardiology. Subject of the test is introducing a Model for observation and treatment of patients with unsatisfactory blood pressure control and the nurses’ role of controlling the risk factors and treatment of high blood pressure. With registered high rates of systolic blood pressure (SBP) initially (162,07mmHg) during the next visitations, a significant reduce is observed, which is most tactile toward the second visitation with an average reduce of 22,48mmHg. The same trends have also been registered with the diastolic blood pressure (DBP), as the results hold the same levels, until the end of the study. This also reflects in a better control of hypertension at the end of the observation.
