Patients allergic to metals and metal compositions represent a serious challenge for the dentist. The heterogeneity of allergic reactions makes diagnostics and solution a complex task. Metal compositions for prosthetic treatment are extremely strong allergenic factor. Well conducted prosthetic treatment involves keeping the prosthetic structure for years. The continuous load of metal ions which is a result of corrosion in the oral cavity, can bring contact allergy to metal compositions. This should always be taken into consideration while conducting treatment of allergic patients especially and in case there are allergic indications, to choose metal-free prosthesis. On the other hand, materials and techniques from other fields of science are becoming increasingly familiar in dentistry. It is recommended not to exceed the indications for use of one material or another and strictly observe technological requirements. 1. In 97% of the cases the removal of structures with metal elements and replacing them with structures of laboratory composites (LC) in patients with known allergy leads to resolution of the allergic reactions. 2. The mechanical properties of LC are slightly changing after prolonged functional period, mainly because of insufficient polymerization, and to a lesser extent, due to the totality of physicochemical and mechanical factors in the oral cavity. 3. The proposed diagnostics model and sanation of patients with known allergy to metals and metal compositions – both on natural teeth and implants, can be useful and applied by dentists. 4. For the most part, the various systems of LC are similar in nature, but only systems with reinforcing fibers provide the necessary strength and elasticity for bridgework on natural teeth and implants. The study of metal compositions used to treat dental crowns defects and dentition imperfections in terms of their allergenic potential, confirms that nickel containing compositions have higher allergenic potential, regardless of the content potential. Early and late sanation effect in 164 patients (62.3% of all surveyed 263 patients) with known allergy to metals and metal composition, confirmed that LC are an excellent alternative to precious metal compositions, ceramics and zirconia. Allergy manifestations in the oral cavity were resolved definitively and without any recurrence in the period of few days to few months after removal of the proven allergens and their replacement with LC structures. The proposed 6 LC constructions provide adequate hypoallergenic prosthetic treatment of patients with allergies to metals and metal compositions in daily dental practice: Monobloc modified overlay 2-parts pin stump of LC reinforced with Vectris Integrated channel splint of reinforced LC Upper implant block of reinforced fiber LC Upper implant removable denture plaque reinforced with Vectris Modified upper implant stump metal-free constructions The proposed technique for extending the functional life of LC structures is easily applicable in everyday dental practice. Patent request registered (PI - 109 793 from 12.01.2007) for Photopolymer Intraoral Camera can greatly facilitate the dentist both in direct repair and in LC cementation.
